      How do I embed the Appt Request form onto my clinic's website?

      1. Open the website editor where your website’s page is hosted

      2. Create a new html page where you’d like the Appointment request form to appear. You can use your page template with your website’s header, footer and other components as usual.

        1. Note: if you're using a website builder (e.g. Wix), you can add a html code block anywhere on the page where you'd like the form to appear. 

      3. Copy and paste the provided code anywhere in the HTML where you would like to show the appointment form.

        1. Note that this will always be inside the <body> tag.

        2. Note that <DOMAIN> in the embed code needs to be replaced with your practice's website.
